Alpha-1-Fetoprotein (AFP) ist ein Glykoprotein, das nur während der Embryonalentwicklung im Laufe der Schwangerschaft vom entodermalen Gewebe gebildet wird.

Unübliche Konzentrationen von AFP im Fruchtwasser und im mütterlichen Blutserum gelten als serologischer Softmarker für körperliche Fehlbildungen und Chromosomenbesonderheiten: Ungewöhnlich hohe AFP-Konzentrationen können z. B. auf eine Spina bifida aperta, eine Anenzephalie oder Leberentzündungen, aktive Hepatitis, Leberzirrhose und Leberzellkarzinom (Hepatozelluläres Carcinom, HCC) hinweisen. Ein unüblich niedriger Wert gilt als Hinweis auf ein Down-Syndrom (Trisomie-21) beim Baby.

Die AFP-Konzentration kann beim Triple-Test bestimmt werden. Allerdings müssen die Werte immer kritisch betrachtet werden, denn die AFP-Werte sind empfindlich: Schon ein ungenau angegebenes oder nicht sicher datierbares Schwangerschaftsalter (Gestationsalter) kann erhebliche Werteschwankungen hervorrufen. Darüber hinaus können erhöhte AFP-Konzentrationen auf eine Mehrlingsschwangerschaft oder ein Chorionkarzinom hinweisen, sodass immer weiterführende Untersuchungen folgen müssen, um eine Verdachtsdiagnose sicher zu bestätigen oder auszuschließen. Der AFP-Wert reicht dafür nicht aus.
