Antithrombin III (AT III) ist ein endogenes Serpin, das die Serinproteasen der plasmatischen Gerinnung hemmt, Thrombin (F IIa) proteolytisch abbaut und an den Endothelzellen die Synthese des t-PA (Plasminogenaktivator vom Gewebetyp) aktiviert.

Antithrombin ist der Gegenspieler des Thrombin. Es wird durch Heparin in seiner Wirksamkeit verstärkt und beschleunigt (ca. 1000-fach). Ein Mangel von Antithrombin III kann also eine Unwirksamkeit des Heparins oder einen hohen Heparinbedarf zur Folge haben. Nach Operationen sinkt das AT III regelmäßig ab. Es ist daher nur eingeschränkt geeignet, eine Verbrauchskoagulopathie nachzuweisen. AT III kann substituiert werden (Anbinex Inj.Lsg., AT III Immuno Inj.Lsg., Atenativ Inj.Lsg.: 1 IE/kg erhöht den AT III-Spiegel um ca. 1 - 1,5%).

Herstellung

Die Herstellung des Antithrombin ist seit 2008 durch gentechnisch veränderte Säugetiere möglich. Die Milch von genetisch veränderten Ziegen aus den Farmen des US-Unternehmens GTC Biotherapeutics enthält 10 gr. des Stoffes pro Liter Milch.[1]
